All benefits eligible employees have the opportunity to change their benefits annually during our open enrollment period. After this period of time, employees are not allowed to make changes to their benefits, with exception to the HSA, unless there is a qualifying event as listed below. Please contact Human Resources via email or phone at 507-786-3068 to determine the necessary paperwork to make the mid-year change.
Change in Family Status
- Birth or adoption of a child
- Marriage or divorce
- Death of spouse or dependent child
- Loss of dependent status (child reaches age limit under the plan or is no longer eligible as a dependent)
Change in Your Employment Status
- Change to or from part time employment
- Beginning or returning from an unpaid leave of absence
- Sabbatical
Change in Your Spouse’s Employment Status
(Resulting in Gain or Loss of Coverage for You or Your Dependents)
- Beginning or ending employment
- Increasing or decreasing hours
Other Changes
- Spouse’s open enrollment period
- Gain or loss of eligibility for Medicare/Medicaid for yourself, your spouse, or your child
Timing
If you have a qualifying event, the change must be made within 30 days of the event. Please be mindful of the processing time involved with making a mid-year change and notify Human Resources as soon as possible to being the necessary paperwork. Changes take effect as follows:
- Birth or adoption of child – they day of the birth or adoption
- Spouse loss of employment – the first of the month coincident with or following the qualifying event
- Dependent child attains the limiting age – coverage ends the last day of the child’s birth month
- Change of marital status – the first of the month coincident with or following the qualifying event
- Spouse’s open enrollment period – the first of the month coincident with or following the qualifying event
