Transportation Waiver<\/h2>\n\n

As the parent or guardian of the minor registering for this St. Olaf College camp, I consent to his\/her participation and I understand and agree as follows:<\/span><\/h3>\n<\/div>\n
    \n
  1. I understand that in the course of the camp or other activity at St. Olaf College, situations may arise where staff may deem it appropriate to have my child transported to a medical provider for observation and treatment.\u00a0 In cases of a medical emergency, it is the policy of St. Olaf College to contact paramedics, and to transport individuals by ambulance as they deem appropriate.\u00a0 In instances where the issue does not appear to be a medical emergency, St. Olaf College personnel are available to provide transportation.\u00a0 In the case of participants in summer camps and activities, this service is conditioned upon receipt of this signed Release of Claims and Indemnification Agreement (the “Agreement”) from a parent or guardian.<\/li>\n
  2. In consideration of my child receiving transportation services to\/from Northfield Hospital or clinic by St. Olaf College personnel (the “Transport”), I, on behalf of myself, my family, heirs, estate, representatives and assignee’s, agree to release and discharge St. Olaf College, its Board of Regents, employees, independent contractors, agents, and all other persons and entities (hereinafter the “Released Parties”) from any and all present and future liability, demands, actions or causes of actions (“Claims”) resulting from the Transport.\u00a0 In addition to the foregoing, I agree to defend, hold harmless and indemnify the Released Parties from any and all Claims released herein which may arise from the Transport, including but not limited to reasonable attorneys fees incurred by Released Parties as a result of Claims released herein.<\/li>\n
  3. I realize that St. Olaf College reserves the right to determine when paramedics should be contacted in lieu of a Transport.<\/li>\n<\/ol>\n\n <\/div>\n\n \n <\/div>\n\n\n\t\t\t\t\n\t\t\t<\/div>\n\t\n\t\t\t<\/div>\n\t\n\t\t\t\n\t\n\t\t\t
