Faculty and staff must complete the High Risk Accommodations Request form. This information will enable Human Resources to work with the supervisor/Associate Dean to find the best way to accommodate the employee. This information will only be collected in Human Resources, which follows the strict HIPAA Privacy Rule on safeguarding confidential medical information. The form will be updated to reflect the most recent CDC guidelines and accommodation requests will be addressed accordingly. If your supervisor has approved a hybrid or 100% remote work accommodation for fall term, due to your own, or a household member’s, valid health condition resulting in higher risk during the COVID-19 pandemic, these accommodations will be extended through May 31, 2021.
