These immunizations must meet the vaccine schedule as defined by the MDH. The two shot MMR series cannot be administered before the 1st birthday or sooner than 28 days apart from each vaccination. The TDAP or TD vaccine must be given within 10 years of matriculation to college. This information must be documented and submitted to Health Services for verification.<\/p>\n<h6><strong>Required Health Forms<\/strong><\/h6>\n<p>Through the\u00a0<strong><a href=\"https:\/\/olecare.stolaf.edu\/\">St Olaf OleCare Portal<\/a>, <\/strong>please read through the directions carefully and submit\u00a0the\u00a0<strong>following REQUIRED forms: \u00a0Immunization data, Tuberculosis (TB) Screening Questions, Confidentiality of Records, and<\/strong><strong>\u00a0Medical History.<\/strong><strong> You will also upload a copy of your immunization record to the portal.\u00a0<\/strong><\/p>\n<p>Students will have access to the portal once they have made an official deposit to the College and have set up a username and password. \u00a0Please allow 24 hours after setting up the username\/password to access the OleCare portal.<\/p>\n<p><strong>You must enter the dates of the required immunizations, upload a copy of your immunization record, and complete the Tuberculosis Screening Form, Medical History, and Confidentiality of Records. Complete all requirements by July 15. \u00a0Immunization and TB requirements not submitted will result in a HOLD being placed on registration.<\/strong><\/p>\n<p>Students, if you are having difficulty using OleCare:<\/p>\n<ul>\n<li>Be sure you are using your St Olaf email and password. \u00a0If you have recently changed your password and you can\u2019t log in, contact the IT help desk.<\/li>\n<li><strong>P<\/strong><strong>hone:\u00a0<\/strong>Ext. 3830 (507-786-3830)<\/li>\n<li><strong>E-mail:<\/strong>\u00a0<a href=\"mailto:helpdesk@stolaf.edu\" target=\"_blank\" rel=\"noopener noreferrer\">helpdesk@stolaf.edu<\/a><\/li>\n<\/ul>\n<p><strong>**Athletes submitting immunizations and other required items to the Athletic Department must still complete the online requirements for Health Services, including the Medical History and Confidentiality of Records.<\/strong><\/p>\n<p>Medical History is not reviewed upon submission, but is reviewed when care is established in Health Service. \u00a0Students need to complete the form before being seen in Health Service for any appointment.<\/p>\n<p><!-- begin-migrated-from-panel-builder --><!-- end-migrated-from-panel-builder --><\/p>\n<\/div>","protected":false},"excerpt":{"rendered":"<p>Welcome Oles!
