The state of Minnesota requires that all students attending college be immunized for or have legal exemption for measles, mumps, rubella, tetanus, and diphtheria. These immunizations must meet the vaccine schedule as defined by the MDH. The two shot MMR series cannot be administered before the 1st birthday or sooner than 28 days apart from each vaccination. The TDAP or TD vaccine must be given within 10 years of matriculation to college. This information must be documented and submitted to Health Services for verification.<\/p>\n

Immunization information is Due July 15.\u00a0 Students who do not provide the required immunization information will have a hold put on their Student Account and will not be able to register. <\/strong>\u00a0 Immunizations are self reported by the student on the Student portal, but students ALSO need to upload a copy of\u00a0\u00a0an immunization record for verification. When submitting the information into the portal, you will need to make sure the dates match the corresponding documentation.\u00a0<\/strong>\u00a0 If you are having difficulty finding documentation of your immunizations, check with your high school or your health care clinic. \u00a0If immunization records are not available, you will most likely need to be re-vaccinated or have titers drawn.\u00a0 Please schedule an appointment with your Health Care provider ASAP to discuss what you will need BEFORE arrival on campus. \u00a0Health Services can review your record if you are unsure how to interpret your immunization record. \u00a0Notify Health Services by email<\/a> if you are having difficulty obtaining immunizations before arrival\u00a0and we can help you explore options for meeting the requirements once you are on campus. \u00a0If you are declaring a conscientious or religious objection to immunization, you must have the printed\u00a0form<\/a> notarized<\/strong> before uploading. \u00a0If you are declaring a medical exemption, please upload medical documentation or have your health care provider mail\u00a0documentation regarding the medical exemption.<\/p>\n
