Report a Workplace Injury or Illness

Step 1

Seek medical attention, if necessary.

Step 2

Notify your supervisor.

Step 3

Print the three documents listed below. Please note: Due to the forms containing private, confidential information, you must print and deliver them to Human Resources in Tomson Hall, room 180. The Employee Information document is for your reference and does not need to be returned to Human Resources.

Step 4

Human Resources will submit the injury/illness information to St. Olaf’s insurance carrier and provide further guidance to you.
