Please complete your Report of Medical History, TB Questionaire, and Immunization information through the St Olaf Student Health Portal no later than July 15th. Your immunization record needs to be reviewed prior to matriculation per MN law. Students who do not provide the St Olaf college required immunization information will have a HOLD placed on their student account. If you are having difficulty finding documentation of your immunizations, it might help to check with your current high school or your health care clinic. If you submitted paperwork to the athletic department, the Report of Medical History, TB Questionnaire, and immunization information still needs to be completed on the portal. Do not submit your athletic physical to Health Service. If you have difficulty accessing the portal, try using a different internet browser. Chrome works best. Our preference is that you use the portal, because that information is entered directly into your electronic medical record, but if you have difficulty getting onto the portal or submitting, print off the forms from the website and mail or deliver during the first week of classes to Health Service. Health Service does not accept them as email attachments or faxes.
Your Report of Medical History will be reviewed only when care is established in Health Service during an office visit.
A physical is not required for completion of the form, but is highly suggested for all students before arrival on campus. Obtaining a physical allows your health care provider an opportunity to update any needed immunizations, refill medications, and assess health needs. Physicals are not available on campus. St Olaf requires vaccination for Measles, Mumps, Rubella,Tetanus (2 doses MMR), and Meningitis (A,C,W,Y). TD/Tdap (tetanus booster) must have been given within the past 10 years. A meningococcal booster must be within the past 5 years of matriculation. The current requirement is a quadrivalent meningococcal conjugate vaccine (MCV4/ACWY) such as vaccines Menomune or Menactra. Meningitis B vaccine, although recommended, is not yet required. Although, there are currently new Meningitis B vaccines on the market, they are not required at this time because they are not yet widely covered by insurance. Obtain current boosters if needed.
We encourage all students to get their flu shot before arrival to campus in the fall, but a flu shot clinic will be on campus in the fall for a fee.
