Health Forms Required for Deposited Students
Please complete the Immunization information, TB Questionnaire, Confidentiality of Records and Report of Medical History located on the St Olaf Student Health Portal no later than July 15th. The Student Health Portal is the preferred way of submitting your information, as it goes directly into your electronic record and can be easily verified. Detailed instructions can be found on the Student Health Portal. Please do not email or fax forms.
