Report Sexual Violence

NOTE: This form may be used by a member of the St. Olaf College community who has either experienced or witnessed sexual assault or another form of sexual misconduct.

The questions provided in this form are optional. All information provided will be forwarded to the Title IX coordinator. The information will be used to track reports of sexual violence for reporting purposes to state and federal authorities. This information will also be used by the college to better understand the incidence of sexual violence in our community, and to aid St. Olaf in its education, prevention, and response efforts. Providing information on this form will neither result in an investigation nor initiate St. Olaf’s complaint process.

Individuals who have experienced sexual assault or another form of sexual misconduct are encouraged to report to a member of the Title IX Team. If the identity of this individual is provided below, the college will follow up directly to offer support services and interim measures and will discuss the option of initiating a complaint under the college’s complaint process. Reporting this information to a member of the Title IX Team does not mean that a complaint must be pursued.
