St. Olaf College Health Services Accommodations Documentation Policy
1) St. Olaf College Health Services will provide documentation for academic, housing, or dining accommodations for mental health or physical disabilities through the Disability and Access Center (DAC) or through an off-campus entity if/when clinically indicated and after the student has signed a release of information.
2) Documentation for accommodations is provided in the context of an ongoing therapeutic relationship with a treating clinician. Students requesting documentation must be actively engaged in a treatment plan for the condition causing disability. The number of appointments it takes to gather all relevant information to provide adequate documentation is at the discretion of the clinician. We do not provide documentation for disability accommodations in the first appointment.
3) Ongoing assessment of symptom severity, frequency, intensity, duration, and functional impact may be recommended. The clinician may require additional evaluations or outside records to clarify diagnoses and the impact of health conditions on functioning.
4) Documentation from Health Services related to disability accommodations does not guarantee accommodations will be granted.
5) For on-campus or off-campus housing, Health Services will not provide documentation if the request does not meet standards for reasonable accommodations, as outlined by the St. Olaf College Disability and Access Center: https://wp.stolaf.edu/academic-success/general-guidelines-on-disability-accommodations-for-campus-housing/
6) Students with existing documentation for accommodations from previous academic years are not guaranteed documentation for the upcoming academic year, as symptoms and their impact on functioning can vary. Providing new documentation for subsequent years is at the discretion of the clinician.
7) Health Services does not provide accommodation documentation to former students or students who are not currently eligible for services due to enrollment status. Former students may sign a release of information if historical records are requested by an outside care provider.
